Your 1st Visit

On average it takes 10-12 minutes to fill out the paperwork at a new doctor's office. If you'd rather spend that time with the doctors, we have a simple suggestion. Rather than struggling to balance a clipboard on your knee simply download and complete the forms listed below under Patient Information Packet before you arrive.  They can be filled out online and printed OR printed and completed by hand. Either way, it allows you to complete this at your convenience. Mail the completed paperwork to us in advance or bring it with you to make your first visit go more smoothly.

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
